The above-named patient was admitted into Life Entabeni Hospital via trauma on
26/08/2024 with complaints of feeling tiredness with the diagnosis of Anaemia (HB 8.6) for
further investigation. He is a known patient with Hypertension, Diabetes on treatment.
History of Right Carotid Endarterectomy (CEA) for Carotid Stenosis in 2021.
The treating physician consulted me on 29/08/2024 to assess the patient and seek my
opinion on the carotids. On examination, he was stable and apyrexial. Radial pulses were
palpable. Abdomen: NAD. Lower limbs were warm. Pedal pulses were palpable.
The clinical diagnosis was Anaemia with a history of Right Carotid Stenosis.
